Patient Information Forms
Complete the Pre-Admission Assessment before your appointment. The day before your surgery, one of our nurses will call you to review this information and answer any questions you may have about your upcoming procedure.

 

 

Please read these general guidelines to help you and your family prepare for your pre and post operative care. Instructions specific to your personal care will be given by your physician.

 

Avoid surprises! Knowing your benefits and expected financial obligations ahead of time will help ease your entire experience. Expect a call from us prior to your procedure to confirm your financial responsibility. Your estimated payment is due at time of service and any balance is due when your insurance has settled your claim. You can pay your bill on line by selecting the “Easy On-line Payment” button on this page and following the steps as indicated. Thank you for timely payment.

 

 

This is our mutual agreement to communicate openly with each other and to address problems timely and appropriately. You will be asked to sign a “Attestation and Consent to Disclose Information” form.
 

 

This is our commitment to respect your privacy. You will be asked to sign a document stating that you have received this information. We will need a listing of persons that we, and your surgeon, can speak to about your procedure.

 

We are required to advise you of the physician ownership in our surgery center.
