Complete the Pre-Admission Assessment before your appointment. The day before your surgery, one of our nurses will call you to review this information and answer any questions you may have about your upcoming procedure.

This is our commitment to respect your privacy. You will be asked to sign a document stating that you have received this information. We will need a listing of persons that we, and your surgeon, can speak to about your procedure.

This is our mutual agreement to communicate openly with each other and to address problems timely and appropriately. You will be asked to sign an “Attestation and Consent to Disclose Information” form.

We are required to advise you of the physician ownership in our surgery center.
Post-Operative Resources
Payment
Your estimated payment is due at time of service and any balance is due when your insurance has settled your claim. You can pay your bill online by selecting the “Easy Online Payment” button on this page and following the steps as indicated. Thank you for timely payment.
